The action takes place in the 1920s in Copenhagen. Illustrator and artist Gerda Wegener asks her husband Einar Wegener to pose as a female model. Portraits become popular and Gerda continues to paint with her husband as a woman. Einar likes his own feminine appearance and begins to live under the name Lili Elbe. Elbe subsequently becomes the first person to undergo gender reassignment surgery.
